For enrollment, follow the steps below:

  • Download and fill-out the registration form. Be sure to fill the form completely. Save the file with the name of the enrollee as excel file or convert to pdf file. (e.g. Juan dela Cruz.xls or Juan dela Cruz.pdf). If you intend to send a pdf file, please preview your filled out and saved pdf form to check that all the information are visible before sending to us.
  • Pay the face-to-face (F2F)/online course or online tutorial/enrichment program you intend to enroll.
  • Send the accomplished registration form together with the proof of payment to [email protected] with the SUBJECT: F2F or Online Course (for review programs), or Tutorial (for A.Y.) or Enrichment (for advance study) – name of enrollee (e.g. F2F CETR – Juan dela Cruz or Online CETR – Juan dela Cruz)
  • Wait for a reply to your email at least a day.
